[ Pastor’s Recommendation ]

Step One:
Fill out a Counselor Application form and mail it to the address at the bottom.

Step Two:
Give the Pastor’s Recommendation form to your pastor and have him/her mail it directly to the address at the bottom. Recommendations that come in with the application will not be accepted.
